Preparing for your Visit:
Here are important next steps to follow after scheduling your appointment:
1. FILL OUT THE REGISTRATION FORM
Please complete our new patient registration forms before arriving for your appointment. Please note that there are 6 pages total.

Submit electronically
Fill out the forms electronically using your PDF software and email it back to us at

Print, then email
Print out the forms and fill them in manually. Then scan them and email to us at
staff@advancedsinusandallergy.com
Print, then fax
Print out the forms, fill them in manually and then fax to (847) 655-7450.
Print, then bring with you
Print out the forms, manually fill them out and bring with you to your appointment.
By completing the forms before your appointment you ensure that the doctor will be able to spend the full appointment time with you.
2. ARRIVE 15 MINUTES EARLY
If you do not fill out the forms ahead of time you will have to complete them when you arrive. THIS MEANS THE DOCTOR WILL HAVE LESS TIME TO SPEND WITH YOU and you may need a followup appointment for testing.
Please arrive 15 minutes before your scheduled appointment time if you did not already complete the registration form.
3. BRING THE FOLLOWING ITEMS
In addition, please bring the following to your appointment:
-A photo ID
-Your insurance card (To view a complete list of accepted insurance plans, Click Here.)
-Prior medical records when possible. (e.g., operative reports, CT scan reports and disks, previous allergy testing results)
4. AVOID CANCELLATION CHARGES
Please notify us 24 hours in advance if you need to cancel or reschedule your appointment. You will be charged a $50 cancellation fee if you don’t notify us 24 hours in advance.
