After scheduling your appointment you will receive an email with instructions on how to sign up for the Patient Portal.
PLEASE COMPLETE THE ONLINE REGISTRATION FORMS BEFORE ARRIVING FOR YOUR APPOINTMENT.
By completing the forms before your appointment you ensure that the doctor will be able to spend the full appointment time with you.
If you do not fill out the forms ahead of time you will have to complete them when you arrive. THIS MEANS THE DOCTOR WILL HAVE LESS TIME TO SPEND WITH YOU and you may need a followup appointment for testing.
Please bring the following to your appointment:

Prior medical records when possible. (e.g., operative Repots, CT scan reports and disks, previous allergy testing results)
IMPORTANT NOTE: Please notify us 24 hours in advance if you need to cancel or reschedule your appointment. You will be charged a $25 cancellation fee if you don’t notify us 24 hours in advance.
