Preparing for your visit:

Here are important next steps to follow before your visit:

 

1. SPEAK WITH OUR STAFF TO CONFIRM YOUR APPOINTMENT

You can book your appointment online but you will still need to speak to one of our staff so they can confirm the appointment time and ask for more details about your symptoms. Dr. Caballero is an Ear Nose and Throat doctor but she ONLY treats disease of the nose and sinuses. We want to make sure that you're making an appointment for the right reasons so you we don't waste your time.  

 

2. SEND A PICTURE OF YOUR INSURANCE CARD

After you have spoken with our staff you will receive a text asking for a picture of the front and back of your insurance card. Please respond to this text within 24 hours so we can properly assess your insurance benefits. Just giving us the Member and Group numbers from your card are NOT enough information. We need to see the front and back of your card. 

 

3. FILL OUT THE ELECTRONIC NEW PATIENT FORMS 

After we have received your insurance card information, you will receive another text (or an email) with a link to our new patient registration forms.  The forms are easy to fill out on your phone, tablet or computer but they are a little lengthy because it provides Dr. Caballero with all the information she needs to access your condition before your visit.

These completed forms must be submitted to us at least 2 days before your appointment. If we do not receive the forms in time your appointment may have to be rescheduled.  

(If you are an established patient you will just receive our Benefit Verification Form)

 

4. BRING THE FOLLOWING ITEMS

Please bring the following to your appointment:

-A photo ID

-Your insurance card (So the physical card can be scanned into our system). 

-When appropriate, bring any prior medical records such as operative reports, CT scan reports and disks, or previous allergy testing results.

 

3. ARRIVE ON TIME

Please arrive on time for your appointment. If you are more than 5 minutes late you may have to wait until the next scheduled patient is seen before the doctor can fit you in. Or, your appointment may have to be rescheduled for another day.  

 

4. AVOID CANCELLATION CHARGES

Please notify us 24 hours in advance if you need to cancel or reschedule your appointment. You will be charged a $50 cancellation fee if you don’t notify us 24 hours in advance.
